Online Forms

At the office of Dr. John Freeman, we offer patient forms online so you can complete them in the convenience of your own home or office. Fax us your printed and completed forms or you may bring them during your visit. There is a $50 fee for new patient appointments. There is q $25 fee for canceling any appointment with less than 24-hour notice.
